Brent Health App allows you to book GP appointments, order repeat prescriptions, access summary medical records, find local NHS services and there is also a symptom checker. Brent Health App is now available for download on Android and Apple devices, please follow the links above.

There are Hub services available over the bank holiday weekend, call 111 to be directed to a hub service if you have an emergency.
Managing Diabetes during Ramadan
Join us for an informative workshop on managing diabetes during Ramadan led by Diabetes Specialist Nurse Nina Patel and Diabetes Specialist Dietitian Salma Mehar.

Patient online access
You can now book appointments online by going to https://patient.emisaccess.co.uk/
To access the service, you need to first be registered for it. You can accomplish this by registering yourself on the above website or by coming into the Practice and asking for a user ID.
Health and social care services in Brent are working so they can focus more on your needs and bring together all the services involved in your care. For the care to be truly built around you, each of the professionals treating or caring for you needs to be able to view selected information about your health, social care packages and -crucially- your personal goals for your care.
All providers of social and NHS care in Brent - social services, local GP practices, NHS hospitals and community services - are letting people know plans to share patient record data and create an integrated care for you. Only people involved in your care will be able to see your records. This means health and social care professionald such as GPs, nurses and social workers along with support staff who organise your appointments or your treatment.
With your permission, we would alos like to share data with health and social care teams discussing your diagnosis, treatment and care at case conferences to provide the best care for you. If you do not wish to share your information, please tell us.
